The Noord-Holland police made multiple mistakes in the investigation into a sexual assault case in Hoorn, the Inspectorate for Security and Justice concluded after looking into the investigation. The victim tracked down the man who tried to rape her herself, NU.nl reports.

According to the inspectorate's report, sent to the chief of police on Wednesday, the police's communication with the victim was not proactive enough and unclear, and the police should have acted more quickly.

Missing was found dead in the woods on holiday park Horsterwold in Zeewolde on Thursday afternoon. Her body was taken to the Netherlands Forensic Institute in The Hague for investigation. At around 9:00 p.m. the police announced on Twitter that their investigation at the scene was done and the area is open to the public again.

The call centers for national emergency number 112 and Ambulance Amsterdam are not up to the task of handling a peak in callers, according to an investigation into the availability of these call centers during a massive power outage in Amsterdam and surrounds in January. This resulted in life-threateningly long waiting times, Het Parool reports.

The investigation was done by the Inspectorates for Healthcare and Security and Justice and the Telecom Agency. The investigators concluded that the availability of 112 falls short during great peaks and must improve.

The approach to fighting contraband in Dutch prisons is not effective enough, according to a report by the Inspectorate for Security and Justice. Prison staff don't have enough time and find it increasingly difficult to keep contraband like drugs and cellphones outside prison walls, the Inspectorate concludes, ANP reports.

The Inspectorate investigated seven prisons. The researchers concluded that prison staff are actively involved in counteracting contraband smuggling and there is no suggestion of a free trafficking market. But improvements are necessary.
